Guidance and Overview

This registration form is used to register for Inhouse Only. It must be completed by the parent or guardian of the player(s). Up to 4 players from the same household can be registered on this form. In order to complete registration using this form, you must make the payment through PayPal. A PayPal account is not required and a small fee is added to the normal registration fee which is automatically added to the total payment due so you are aware of the total cost. It is highly recommended that you read the 2021 Registration Flyer on this web site before completing this form. A late fee of $50 is added for all registrations starting April 2, 2021.

Liability Waiver

As a participant in the Limestone Youth Baseball League (the League) Program, I recognize and acknowledge there are certain risks of physical injury present, and I agree to assume the full risk of any injuries including death, damages or loss which may be sustained by my child as a result of participation in any and all activities connected with or associated with the Program.  I agree to waive and relinquish all claims I may have as a result of my child’s participation in the Program against the League, its officers, agents, servants and employees from any and all claims for injuries, including death, damage or loss which I may have or which may accrue to me on account of my child’s participation in the Program. I further agree to indemnify, hold free and harmless the League, its officers, agents, servants, and employees from any and all claims resulting from injuries, including death, damages and losses sustained by my child and arising out of, connected with, or in any way associated with the activities of the Program. In case of accident or sickness, I consent to emergency medical care provided by ambulance or hospital personnel.
